What you need to do:

• Fill out the Medical Form form and email, fax, or mail it back to us.
  
  
• Fill out the Arrival Details form on line and email it back to us.
  
  
• Print out the Contract Form, fill it out and email, fax, or mail it to us.
  
  
• Print out the Waiver Form, fill it out, and email, fax, or mail it back to us.
